The Hoop It Up  Official Team Roster form must be filled out by all teams participating in a Hoop It Up tournament .  This form must be handed in to a Hoop It Up staff member prior to their first game.  Teams that fail to hand this form in will be disqualified from the tournament.  No refunds will be issued to teams that are disqualified from a tournament. 

Please note:  This form requires the signature of the parent or legal guardian of anyone under the age of 18 so should be filled out prior to check-in.
